Submit a referral form for Care Management or CFTSS services. If you’re unsure which service you need, contact us and we’ll guide you.
2. Intake
Our Intake team will reach out to learn about your situation and explain our programs. We'll help determine which services would benefit you most.
3. Assessment & Eligibility
Our licensed clinicians will evaluate your needs and verify insurance coverage. Evaluations can take place wherever you're most comfortable - in your home, community, or at our office.
4. Provider Match
We'll carefully match you with the right provider based on your specific needs. We consider factors like language, cultural background, gender preferences, and location to ensure the perfect fit.
5. Services
Once matched, services begin in the setting of your choice - whether that's your home, in your community, or at our facilities. Your comfort and convenience are our priority.
6. Ongoing Support
Your provider maintains consistent communication throughout your care journey, with regular check-ins based on your needs. We're here to support you until your goals are met or circumstances change.
