Our Beginning…
In 2018, JEMCare was formed to address a critical need within underserved communities. Many individuals and families were struggling to navigate the complex healthcare system, facing significant barriers that led to unnecessary hospitalizations and gaps in care.
JEMCare is designed to help people with complex needs access the care and services necessary to stay healthy. Built on the belief that families should have the guidance they need to successfully access essential support services in their own language, JEMCare works to ensure everyone receives the care they deserve.

We built our foundation on trust, fostering stable, long-term relationships with our members. This approach allowed us to truly understand our communities' needs, enabling us to thoughtfully expand from a care management agency into a full-service provider offering a range of programs to best serve our members.
