Home and Community-Based Services
HCBS provides intensive support for children and youth at risk of hospitalization or out-of-home placement, working closely with Care Management to strengthen families, improve coping skills, and address core challenges—promoting long-term stability and preventing more severe interventions.

FAQs
What locations are covered with JEMCare HCBS?
Who can make an HCBS referral?
Referrals for HCBS services must come through a Care Management agency or C-YES. Schools, healthcare providers, and other organizations cannot refer directly.
Does JEMCare offer HCBS services for adults?
Currently, JEMCare’s HCBS program is only available for children and youth up to age 21. An adult HCBS program is planned for the future.
Where are HCBS services provided?
Services are provided in your home or community settings, based on where your loved one feels most comfortable.
Can HCBS services take place during school hours?
No, they cannot.
How long do HCBS services last?
Services continue as long as they're needed and the participant remains eligible. This could be until goals are met, circumstances change, or they age out of the program.
Are there any costs for HCBS services?
No, services are fully covered for individuals with Medicaid.
