Community Oriented Recovery and Empowerment
CORE services help adults living with mental health challenges build skills, increase independence, strengthen community connections, and achieve their recovery goals through personalized, community-based support.

Who is eligible for CORE?
Adults age 21+
Enrolled in Medicaid
Diagnosed with a qualifying mental health condition
Eligible for CORE services through their Medicaid Managed Care, HARP, or HIV SNP plan
CORE Services
Psychosocial Rehabilitation (PSR)
Supports individuals in developing daily living, social, educational, and vocational skills necessary for successful community living.
Community Psychiatric Support and Treatment (CPST)
Goal-oriented interventions focused on mental health recovery, symptom management, and overall wellness.
Peer Support Services
Recovery-focused support provided by trained peers who use their lived experience to encourage and empower participants.
Family Support and Training
Education and guidance for family members and natural supports involved in an individual's recovery journey.

Where are CORE services provided?
Services are provided in the home, community, or other settings based on individual needs.
Who can make a referral??
Self-referrals, family members, providers, hospitals, care managers, and community organizations.
How long do CORE services last?
Services continue as long as they're needed and the participant remains eligible. This could be until goals are met, circumstances change, or they age out of the program.
Are there any costs for CORE services?
No, services are fully covered for individuals with Medicaid.
