Who can make a Care Management referral?

What's the difference between child and adult Care Management?
While both programs provide similar support, they're tailored to different age groups. Individuals between 18-21 can choose which program better fits their needs.
Where are Care Management services provided?
Services can be provided wherever is most convenient for you - in your home, community settings, or at our facilities.
How long do Care Management services last?
Services continue as long as they're needed and the participant remains eligible. This could be until goals are met, circumstances change, or they age out of the program.
Are there any costs for Care Management services?
No, services are fully covered for individuals with Medicaid.
Can I receive Care Management services along with other services?
Yes, Care Management services can complement other treatments and support you are already receiving.
